Does any of this sound familiar?

Ready to move out, but a group home doesn't suit your needs?

Could live on your own, but just need some support/supervision?

Too much turn over of community and respite workers?

Want to have a choice in choosing your staff?

Want more choice of appropriate programs?

Then Self-Determination may be for you.

What is Self-Determination?

It is a program funded by OMRDD (Office of Mental Retardation and Developmental Disabilites).  If you are enrolled in (or eligible) for Medicaid Home and Community Based Waiver, then you may tap into this innovative program in NY State.

It is an idea that came from people with developmental disabilities and their families.  They wanted more control over their supports and services.

It's about personal choice, you, together with friends and family decide what you want to do, what supports you'll need and who to hire to assist you.  You arrange your own service schedules.  You authorize payments for supports and services you have chosen.  You have to agree to budget wisely, but you are in control.

A Start-Up Broker is someone who writes the personal plan that navigates you through your life.  Sally Colletti has written over 84 plans for individuals with all types of developmental disabilities.  A support broker is the person who supports the plan once it is approved by NY State.  Advocates For Autism offers these services and is proud to have been a part of over 84 individuals with developmental disabilities

Not sure if you are enrolled?  What to do next? Call today and we can start the process with you over the phone.  (607) 754-9694 or email:  info@advocateforautism.com
